A webform (web form or HTML form) allows one to collect user data/information online. Forms can resemble paper or database forms because web users fill out the forms using checkboxes, radio buttons, or text fields.
Google forms is part of Google Drive.

Formstack can be used on PC, Mac, iOS or Android (computer + mobile devices).
Formstack integrates web apps to make your online forms work for you. With these tools, you can automate data collection or update email subscribers. Just choose a category to get started with form integrations.

Otis College is providing access to a web-based form builder, Formstack, to support in their day-to-day college-related tasks.
Note: Webforms may also be requested and built through Drupal by contacting the Webteam.
Formstack is a service currently offered to staff only at Otis College of Art and Design.
If you have an expressed college-related need for a webform, please submit a request and the Instructional Designer will work with you.
If you wish to create a webform to use for your class, I suggest trying Google forms.
